Comprehensive Eye Care at the Midwestern University Eye Institute

The Midwestern University Eye Institute stands as a leading provider of comprehensive eye care, offering a wide array of services to the community. Located at 59th Avenue and Utopia Road in Glendale, the Eye Institute is committed to delivering personalized and advanced vision correction solutions through state-of-the-art technology and a team of experienced professionals.

Scheduling an Appointment

The first step in becoming a patient is to schedule an appointment by calling 630-745-4500 or 623-537-6000. The Eye Institute accepts many vision plans, and there are options for all budget ranges. For plans in which they do not participate, claim forms can be provided for the patient to file.

Patient Care and Policies

Patient Rights and Access to Care

Every patient will receive a copy of the Patient Bill of Rights at the time of service. The patient shall have access to emergency, incremental, and comprehensive care as appropriate for the patient’s presenting condition(s). Care is provided during posted clinic hours, and emergency consultation is available by phone or, if deemed necessary, treatment at a designated location after regular clinic hours. Treatment is supervised by faculty and is consistent with the standards of care in the community.

Childcare Policy

Children must be supervised by another adult in the waiting area at all times. If children are left unattended, the appointment will be cancelled and rescheduled. Therefore, please make prior arrangements for appropriate childcare.

Cancellation Policy

Appointments should be cancelled only when it is absolutely necessary. For cancellations, 48 hours of notice is required for an appointment cancellation at 630-745-4500 and 24 hours of notice is required for an appointment cancellation at 623-537-6000. Your student doctor will give you the clinic phone number to call directly. Cancellations with less than required hours of notice of your scheduled appointment, it would be considered a “no show” appointment. Additionally, patients who arrive more than 15 minutes late will be considered “no show” appointments and may have to reschedule.

Discontinuation of Treatment

Patients whose treatment is discontinued will be notified in writing and alternate treatment options may be suggested.

Patient Records

At the time of admission as a patient, an individual patient record will be established.

Payment and Insurance

Payment for office services, co-pays, and deductibles are due at the time of service. Payment for optical orders (frames, lenses, contacts, rehabilitative devices) are due in full when the order is placed. The Eye Institute accepts cash, personal checks with valid identification, Visa, MasterCard, Discover, and American Express. There is an additional $30.00 charge for returned checks. If you are unable to pay for your services on the date of your appointment, your appointment will be rescheduled.

If you have vision or medical insurance, please bring your insurance information with you to your first appointment. This should include a valid insurance card, an address for submitting claims, and the name, date of birth, and social security number of the subscriber. The Eye Institute will file your insurance claim for you as a courtesy if you are covered by one of the plans with which their practitioners participate. If their practitioners do not participate in your plan or you do not have insurance coverage, your payment in full for services and products will be due upon check-out from the clinic. It is your responsibility as the patient to verify with your insurance whether you need a referral or prior authorization.

Financial Assistance

The Eye Institute offers a program to assist the public in times of financial hardship. The Eye Institute will attempt to make payment arrangements when necessary. These forms are not intended for emergency referrals.

Comprehensive Eye and Vision Care Services

The Midwestern University Eye Institute offers comprehensive family eye and vision care, including:

  • Ophthalmology

These services are provided by faculty practitioners and specialists in partnership with third- and fourth-year students from the Midwestern University Arizona College of Optometry.

Disease Detection and Treatment

As a person ages, the risk for development of common eye diseases increases. Many eye diseases have no early warning signs or symptoms, but a dilated eye exam can help detect common eye diseases such as macular degeneration, glaucoma, cataracts, diabetic eye disease, and thyroid eye disease. The Midwestern University Eye Institute has state of the art equipment for a variety of eye conditions. Every primary care eye exam at the Midwestern University Eye Institute screens for eye disease.

If you are a patient in our clinic, you will be examined by a team of Midwestern University students and doctor. The exam will be around two hours in duration and you will most likely have your eyes dilated. If you or your doctor suspect an eye disease, additional eye tests may be necessary to assess the health of your eye.

The Vision Center

The Vision Center within the Eye Institute is located on the first floor of our clinic. The Vision Center provides service for all eyewear related needs and we have frames with the latest fashion trends and lens products with the newest technology to ensure you look and see your best. The Vision Center hosts an annual trunk show event where we invite vendors in for exclusive frame selection and discounts on products for our patients.

Optical Services

A patient does not need an appointment to utilize our optical services. A team of Midwestern University students and licensed optician will review the glasses prescription to ensure the frame and lenses selected will best meet your needs. They will assist with frame selection to find a frame out of our vast inventory that best fits your individual face structure and personality. Once the frame is selected, lens design will be reviewed to ensure the best vision and eye comfort with the glasses. The optician will review any vision plan in order to maximize benefits. The Eye Institute contracts with lens labs of the highest quality and once the order is placed, most glasses are ready to be picked up within two weeks of order. We recommend that the patient pick up his or her glasses so we can adjust the frames for the best fit.

How the Vision Center Can Help

The Vision Center has a team of licensed opticians who can assist you in selecting the most appropriate frames and lens technology for your individual needs. Many of our lens products are designed to provide optimal eye comfort due to computer related eye stress. Being in Arizona, our doctors and opticians focus on the need for quality UV protection in both clear lenses and prescription or non-prescription sunwear. Additionally, our opticians provide complimentary adjustment and eyeglass cleaning services to our patients.

Refractive Suite

Advanced Vision Correction Solutions

The Midwestern University Eye Institute announced the opening of its new refractive suite, featuring cutting-edge technology designed to deliver advanced vision correction solutions.

The new suite is equipped with the latest industry-leading devices, including the ZEISS Visumax 800, the Alcon Wavelight 500, and the ZEISS SMILE Pro, expanding the range of corrective options available for Eye Institute patients.

With these new additions, the Eye Institute can now offer individualized, minimally invasive, and advanced services, including LASIK, Photorefractive Keratectomy (PRK), Small Incision Lenticule Extraction (SMILE), and Implantable Collamer Lenses (ICL). These procedures are tailored specifically to individual patients' needs and address a wide range of refractive errors, including myopia (nearsightedness), hyperopia (farsightedness), and astigmatism.

Ophthalmologists from the Arizona College of Optometry (AZCOPT) will provide specialized services in the new suite. Robert Fintelmann, M.D., Associate Professor, is a board-certified and fellowship-trained cornea surgeon with the highest surgical qualifications and will perform the specialized surgical procedures.
