Achieving Fellowship in the American College of Surgeons: A Comprehensive Guide

Board Certification and Practice Experience

Applicants for Fellowship must have been in an independent practice for a minimum of three (3) years (on or before December 1, 2023) and be board-certified.

Waiver Requirements

Please note: If you are NOT certified under the American Board of Medical Specialties (ABMS) and meet one of the following three (3) scenarios, you will need a waiver before applying. Once the waiver application has been forwarded by AAOMS, the ACS Membership Department will begin the definitive fellowship application process. Please note: Acceptance of a waiver does not guarantee fellowship in ACS.

Core Requirements for Fellowship

Several key requirements must be met to be considered for Fellowship in the ACS:

Practice and Specialization

A current practice that establishes the applicant as a specialist in surgery. One year of surgical practice after completion of all formal training. Additional practice time may be required if the practice situation and/or geographic location changes. The degree to which a practice must be restricted to the specialty is to be determined by a responsible College Credentials Committee.

Professional Qualities

Interest in pursuing professional excellence both as an individual surgeon and a member of the surgical community. Ethical fitness as well as professional proficiency as determined by an appropriate College Credentials Committee.

Letters of Recommendation

Three letters of recommendation from Fellows of the American College of Surgeons.

Surgical Case List

A consecutive 12 month listing (from the date of application) of the procedures performed as a surgical attending with responsibility for your portion of the patient’s care. The surgical log will be evaluated based on volume and scope. Specific instructions for the surgical list are below. Use an Excel spreadsheet format that can be electronically submitted.

Application Fee

A nonrefundable application fee (in U.S. dollars) online at the end of the application. This payment is required before you can submit your completed application. Applications will not be processed without payment of the application fee.

International Application Fees

Initiating the Application

Thank you for choosing to apply for Fellowship in the American College of Surgeons. We are here to support you through the application process. Apply now and you will be considered for induction into the College as a Fellow in 2027.

Associate Fellowship

You will become an Associate Fellow of the College while your Fellowship application is being processed.

Surgical Case List Submission

This will include information on when to submit your Surgical Case List reflecting the type and volume of surgical cases you have performed during the past year.

AAOMS Collaboration for Oral and Maxillofacial Surgeons

AAOMS and the American College of Surgeons (ACS) have worked together to establish a new pathway that allows eligible single-degree oral and maxillofacial surgeons to apply for ACS Fellowship. AAOMS is now offering assistance with case log review for dual-degree OMS applicants. Dual-degree surgeons will still directly apply to ACS. The change merely provides case log review for dual-degree applicants. Interested OMSs must submit proof of having completed the following requirements directly to AAOMS. A committee of AAOMS fellows will review all submitted materials and provide ACS with confirmation of the applicant’s eligibility.

The AAOMS Parameters of Care recognizes the following major surgical categories as contemporary scope of practice for oral and maxillofacial surgery. Examples are intended as general guidelines and not meant to be a complete listing for each category.

AAOMS Review Process

Upon receipt of the application and documentation, the AAOMS designated committee will review your request. If complete, AAOMS will forward your application to the ACS Member Services Director.

ACS Review and Final Decision

All ACS applications will be reviewed following the College’s normal Fellowship evaluation procedures. The ACS Board of Regents makes the final decision on eligibility for Fellowship. Please note: Acceptance of the initial application by ACS does not mean a potential applicant will be approved for Fellowship.

Induction and Membership

After you are inducted as a Fellow, you will be billed for dues in January to maintain your membership for the year. The FACS designation can only be used after you become Fellow of the American College of Surgeons.
