The Novo Nordisk Patient Assistance Program (PAP) exemplifies this commitment. The Novo Nordisk Patient Assistance Program (PAP) is based on our commitment to our patients. Patients who are approved for the PAP may qualify to receive free medication from Novo Nordisk. It provides crucial support to eligible patients who may otherwise struggle to afford their diabetes medication.

Eligibility Criteria for PAP

To qualify for the Novo Nordisk PAP, individuals must meet specific criteria:

  • Be uninsured or have Medicare. Uninsured patients are enrolled for 12 months.
  • Have a total household income that is at or below 400% of the federal poverty level (FPL).

Application Process

Applying for the PAP is designed to be straightforward:

  • Applying online is a step-by-step process that tells you what to do next. After you are finished with the form, we'll send it to your health care provider.
  • Note: After the application is reviewed, you and your health care provider will be informed of the decision.
  • If an application is submitted with all supporting documentation and all required fields completed, it will be processed within 2 business days.

Note: The digital form is not available to residents of Maine and Minnesota. Residents of Maine and Minnesota must apply by paper. Maine and Minnesota residents: The online form is not available in your state. If you believe the income is incorrect or have questions, you can supply proof of income yourself.

Enrollment and Medication Delivery

All new applicants will be automatically enrolled. The medication will be shipped to the prescriber of an approved enrollee/applicant in accordance with current program guidelines. Auto refills terminate at the end of the patient’s program enrollment period. Please note: Not all medications are eligible for auto refill.

Reapplication

Patients may submit a new application 30 days prior to the end of their enrollment. Those with Medicare Part D coverage may apply for next year’s PAP enrollment after October 15, {currentYear}. Yes. Maine and Minnesota residents: You must reapply by paper.
